PLEASE NOTE: The reports show the last 24 months of data based on the employee Due Date. For example, if it is January 1, 2023, the reports will show data where the employee Due Date is January 1, 2021 through January 1, 2023.
The Forms report allows Team Members to quickly see the status of their employees’ forms. This report will have a row for every form assigned to the employee or an HR user as a part of the process flow. This report does not show forms that are a part of process flows that have been cancelled or archived.
If the process flow utilizes process chaining or process automation (proc auto), it will only show the forms when they are officially assigned to a employee. This is when the new process is initiated. In the case of skip logic, if the form should be skipped based on skip logic, then the form will not show in the report.
This report shows:
- Name – this is the Employee’s name.
- Display – Last, First Middle
- Sorting – Text by Last, First, Middle
- Filtering – Text using Contains
- User API Identifier – this is a Click Boarding unique identifier for the employee.

- Candidate API Identifier – This is the identifier that is used in employee API calls such as the Candidate/{id} – GET.

- Process Flow – this is the process flow name that the form is a part of.
- Display – Process Flow Name
- Sorting – Text by Process Flow Name
- Filtering – Pick list with text to filter list
- Form Name – This is the Form Title of the form.
- Display – Form Title
- Sorting – Text by Form Title
- Filtering – Pick list with text to filter list
- Status – This is the status of the form. A form that has been reset but not resubmitted by the employee will show as Recertified. If an employee has declined a form in a step, the status will show as Needs Attention.
- Display – Not Started, In Progress Completed, Recertified, Needs Attention
- Sorting – In designation: Not Started, In Progress Completed, Recertified, Needs Attention
- Filtering – Pick list with text to filter list
- Assigned To – This is who the form is assigned to. If the form is assigned to the employee, the employee’s name will appear in this column. If the form is assigned to a specific Team Member, the Team Member’s name will appear in this column. If the form is defaulted to be assigned to the HR Team, HR Team will appear in this column.
- Display – Last, First Middle OR HR Team
- Sorting – Text by Last, First, Middle
- Filtering – Pick list with text to filter list
- Due Date – This is the date the form is due. For employee facing forms this is the employee Due date. For HR facing forms this is the Due Date
- Display – mm/dd/yyyy
- Sorting – Date
- Filtering - Date using Equals, Greater than, Less than, Not equal, In range Blank, Not blank
- Completion Date – This is the date the form was completed. If the form is not complete, it will be blank.
