On the Forms & Content tab of the Processes & Content page, click Create Form or Content.


Click Data Entry.

Enter a Reference Name. This is a unique name to be used internally. It is not candidate facing.
Enter a Form Title. This is visible to the candidate.
Enter an optional Form Subtitle. This is visible to the candidate.
OPTIONS:
- Click Add Content to add an Image, Video, or Text.
- Click Collect Information to create forms that collect candidate information.

Click Add Content to add an Image, Video, or Text.
ADDING TEXT
Click Text.
ADDING IMAGE
Click Image.
ADDING VIDEO
Click Video.
COLLECT INFORMATION

Address: Click to add Home, Work, and Mailing Address related fields in Click Boarding to be added to a form.
Company Information: Click to add custom fields made by our technical team or consultant.
Education, Certification, and Licenses: Click to add pre-existing Education, Certification, and License information. (Field clusters allow for multiple records to be collected).
Government Issued ID: Click to add pre-existing SSN field (US only)
Name: Click to add pre-existing Legal Name related fields in Click Boarding to be added to a form.
Personal Information: Click to add pre-existing date of birth field.
EXAMPLE
Click Company Information.
Select the type of company information to collect. Scroll down and click Next. The Collect Company Information page opens.
Under Collect As, there are three options to configure how you want your field to display:
- Single line of text
- Multiple lines of text
- Selection from list
Single line of text, Multiple lines of text, and Selection from list options allow you to configure:
Field Label: Text to the left of the field that is collecting information
Placeholder Text: Directional text the candidate sees directly in the field that is collecting information
Is this field required?: Consider your demographic when determining if this is required
The Selection from List option contains additional configurations.
Display Field Options Alphabetically: Choose how you would like to sort the field options; manually or alphabetically.
Pro Tip: To quickly change the order of the options, click Bulk Edit and copy/paste the options to define your manually sorted list.

Value: The code for this value that is passed in an integration
Display Label: The display value for this field to the candidate. If this is not completed, the Value will be shown to the candidate.
Add Option: Allows you to add additional selections for the candidate to be exposed to
