On the Forms & Content tab of the Processes & Content page, click Create Form or Content.
Click Document Upload.
Enter a Reference Name. This is a unique name to be used internally. It is not candidate facing. It can not be changed.
Enter a Form Title. This is visible to the candidate.
Enter a Form Subtitle. This is visible to the candidate.
Enter Upload Instructions.
Click Yes or No for Is a file upload required? In most instances, you will want to click Yes, to make uploading a document required. This ensures that the document upload is not inadvertently missed.
Click Create.
You can go Back to Forms & Content (leave the form as a draft), Publish Draft, or Continue Editing.
